Core & Community Team Expectations

 
  • You have filled each role for your Core Team, including: Core Team Lead, Data Lead, two YYA with lived experience, ACI Coordinator, Child Welfare, Juvenile Justice, Behavioral Health and Schools Representatives

  • Your Community Team meets at least once a quarter

  • The Data Lead is submitting monthly data to ACI

  • The Team Lead has scheduled one-on-one check-ins with their Coach to review high-level data and project progress

  • The Team Lead has scheduled bi-monthly meetings with the Core Team to determine what actions we need to take next in pursuit of our goals

Change Ideas

  1. Have a liaison or liaisons [ideally includes big red ball holder] attend leadership meetings and front-line meetings [could be case conferencing] to ensure flows of communication are happening between both groups

  2. Front-line meetings have a standing agenda item soliciting input on barriers that are being faced and generating ideas for solutions. These are shared with the leadership team or teams to help clear the path

  3. Decisions and system problems identified at leadership meetings are shared with front-line staff to keep everyone on the same page and solicit feedback on potential solutions
