Ⅱ. Identify an area of focus
Guiding Questions:
What stands out in the data? What trends do you see?
What are you curious about? What do you think is happening?
Where do you see an area for improvement?
Ⅲ. Decide on a Change Idea
Guiding Questions:
What are we trying to accomplish? What change do we want to see?
What small test of change can we try?
Why do we think this change will solve the problem?
Things to consider:
How are we addressing disproportionality?
Have we asked young people about this idea?
Ⅳ. Plan your Test
Determine how you will measure:
How will we know this change was an improvement?
What data do we need? How will we track it?
Set a specific goal:
By doing [change idea], we will [reduce, increase, etc.] [measurement- housing placements, exits to inactive, active unsheltered, etc. ] by [X amount] in [X days, months]
Does this goal excite us?!
Assign roles and follow-up:
Do you have the right people in the room to execute this plan?
Who will do what? By when?
When will we check in about progress? When will the test end?
